In addition to direct billing of several insurers, we give all clients receipts and universal claim forms for compounded prescriptions. This form includes all the information needed for your insurance to reimburse you.

We are able to bill several local insurances on your behalf, including but not limited to:

• ODS
• HMA Peace Health of Oregon
• Regence Blue Cross/Blue Shield
• OEA Choice
• Lane Individual Practitioners Association (LIPA), with a prior authorization
